Case Management entails the coordination of services for foster children placed with the agency. All calls from the County of Sacramento (and other nearby counties) indicating a needed placement for a child are fielded by an Intake Specialist at which point appropriate foster families are sought and then a placement made. An initial assessment is then conducted by the Intake Specialist during the first three weeks of placement. The case is then transferred for on-going case management to an assigned case manager.


Continued assessment of the child’s needs is conducted by a case manager and weekly contact is made with the child. A plan of services to meet the needs of the child is put in place and followed up on by the case manager. All supervised visits are arranged, and many times monitored, the agency case manager with biological family members. Additionally, the case manager provides support to the foster parent providing daily services to the child.


HELPS is committed to assisting foster youth prepare for the time when they are no longer in foster care. This is done not only through individualized Emancipation Plans, but also through conducting a Life Skills & Planning (LSP) Series each year.
